BridgePoint Supportive Care assists individuals facing the challenges of serious illness and our focus is on your pain and symptom management and family support. This is called
palliative care, a type of care focused on providing comfort and promoting quality of life. It can be accessed while still receiving aggressive treatment. Our main goal is to keep you or your loved one in the comfort of their home.
The Staff of BridgePoint Supportive Care:
- Works with your doctor to manage pain and other symptoms
- Clarified your goals for care and quality of life and explores treatment options
- Assists you in executive advance directives such as a living will
- Assists you in navigating the healthcare system to acquire the care you need
- Links you with community resources such as home care, medical equipment vendors, financial assistance and other services.
BridgePoint Supportive Care consultation visits can be made to a patient living at home, in an assisted living facility or in a nursing home. Adult patients with any illness may benefit. Visits are made by a board-certified physician or a board-certified nurse practitioner with expertise in palliative care and geriatric medicine.
Services provided by BridgePoint Supportive Care are reimbursed by Medicare and many private insurance companies.
